I request to receive a Sportsbet Cash Card and I agree to receiving disclosures about this card online. I understand that Product Disclosure Statement and Financial Services Guide is available online and further information on how to activate my card will be sent to me with my card. I have read and agree to be bound by the Terms and Conditions set out in the Product Disclosure Statement and declare that the details that I have provided in relation to this card application are true and correct. I acknowledge that supplying my mobile number or email address means Sportsbet may use it to contact me, including:
(a) for further information relating to my card or to advise its progress;
(b) if Sportsbet suspects fraud on my card;
(c) for account administration; and
(d) about special offers in relation to products and services (which may be supplied by Sportsbet) which Sportsbet thinks may be of interest to me, unless I unsubscribe from these communications at any time by clicking the unsubscribe link provided on the communications.
